What to Expect

At onset, our team will conduct a comprehensive patient assessment (review of diagnoses and health history, mental capacity, medications, identify clinical needs, challenges, family dynamics, etc.). We identify barriers and necessary services, as well as best treatment options to help stabilize patients and or improve their overall condition and functionality.  This process allows us to develop an appropriate care plan, and help our patients achieve better quality of life.

In addition, our team will:
  • Review each patient's plan of care at regular intervals, make adjustments when necessary or when there is a change in condition, hold case conferences for our team to collaborate and give input, and communicate regularly with the patient, family,  doctors, and all members of the healthcare team
  • Assist with placement and transitions to and from a higher level of care
  • Guide, make recommendations, set-up or help patients apply for community-based or veteran programs and services to enhance quality of life
  • Assist with and order durable medical equipment (DME) to include front wheel walkers, four-wheel walkers, wheelchairs, hospital beds,  communication devices, etc.
  • Educate and train patients, families, and caregivers
